Terms of Use General Policy and Procedures

NEVER USE the Patient Portal to communicate (i) an emergency, (ii) an urgent situation or (iii) sensitive information (e.g. HIV, Mental Health, excuses from work, etc...)

Appropriate Use for the Portal:

  • Use the Patient Portal for questions related to the medical portal; laboratory results, select reports and recommendations.
  • Use the Patient Portal to update your demographic information.
  • Be sure that all information you provide is true, accurate, complete and updated each time you make a change.
  • Be concise when writing a message.

The Patient Portal Offers the Following Features:

  • Send and receive secure emails for non-urgent needs.
  • View laboratory results that have been sent to you.
  • View and print “The Continuity of Care Document” or “The Health Summary.”
  • View and admit new health information.
  • View health information (allergies, medications, co-occurring problems, past visit history).
  • Update demographic information (i.e. address, phone number, etc…)

Communications May Be Part of the Medical Record

Communication through the Patient Portal may be included in your medical record permanently.

Privacy:

  • Any message sent to you through the Patient Portal will be secure (Encrypted).
  • Any email from you to any operator must be through the Patient Portal or they will not be secure.
  • For minors who are not emancipated, parents will have full access to the electronic file. If the minor is emancipated, he or she will have to go through our facility to complete the HIPAA documentation.
  • All email lists will be kept confidential; and the lists will not be shared with others, unless it is necessary to carry out the operation of the patient portal (e.g. updating and improving the Portal) or required by Law.
  • If any situation occurs with the patient's account, verbal authorization will be requested to create a remote virtual session that allows evaluating what happened with said account.
  • A variety of healthcare and administrative personnel (such as nurse practitioners, physician assistants, graduate nurses, certified medical assistants, clerks, etc....) will be involved in reading, responding to, and processing your messages and information provided through of the patient portal (similar to how telephone communication is handled). There is no need to notify us that you have read a message, unless you have a question or need additional information.

  • If you have any concerns about what has been said, please contact the Medical Records Department at 787-834-6000 exts: 3029 and 3030.

Response Time:

  • After you have signed your agreement to comply with the terms and conditions of use for the Patient Portal you will be sent a welcome email. This will provide a link to the patient entry to access the screen. If you have not received an email from us within three (3) business days, please contact the Medical Records department at 787-834-6000 ext. 3029, 3030 or notify the receptionist.
  • Reasonable efforts will be made to respond via email within one (1) business day, but no later than three (3) business days, of receipt. Response time may be longer if the Patient Portal service is interrupted for maintenance, upgrades, or emergency repair related to events beyond our control. In this regard, you agree not to hold Bella Vista Hospital staff, such as its medical practitioners, suppliers or any of its staff in any way responsible for any such modification, suspension, or distribution of patient entry.
  • The Patient Portal is verified during business hours (7am to 3:30pm, Monday to Friday). You have the right to use the Patient Portal at any time; However, any message sent after operating hours will be retained until we can respond to it the next day.
  • If the email is not accessible for any reason, please contact the Medical Records Department directly at 787-834-6000 ext: 3029, 3030.

Medical Advice and Disclaimer Information:

  • The Patient Portal Entry may from time to time include information published by Bella Vista Hospital in the form of news, opinions, or general educational materials that should not be construed as specific medical advice or instructions from Bella Vista Hospital.
  • Patient Entry Information is required to be used for medical diagnosis or treatment.
  • The information published by Bella Vista Hospital on the Patient Portal should not be relied upon in its entirety, nor should it be relied upon to suggest a course of treatment for a particular individual. You should always seek the advice of your doctor with any questions you may have regarding a medical condition and you should never disregard medical advice or delay seeking it because of something you have read in the Patient Portal.
